Financial Policy
We ask that you read the following statement carefully. We must emphasize that, as a dental care provider our relationship is with you, the patient, and not your insurance carrier. As a courtesy to you, we will file your claims with your insurance company. However, you are the sole responsible party for all charges incurred and guarantee payment in full. If we are contracted with your insurance company, we will accept assignment. You will be responsible for your payment portion at the time of service. All co-pays,co-insurance, and deductibles are due and payable at the time services are rendered.
Our office partcipates with Delta Dental, Anthem BCBS, and United Concordia.
If a patient is without dental insurance and financial arrangements need to be made, we do offer Interest-Free payment plans through an outside source called Care-Credit. The application process is easy and they can offer payment arrangements for up to 18 months. www.carecredit.com
We accept cash, checks, Discover,Visa,MasterCard and American Express.
Cancellation Policy
If you are unable to keep an appointment, we ask that you kindly provide us with at least 24 hours notice. We ask for this advance notice so that we can offer this appointment to another patient. A fee may be charged if a patient does not show up for an appointment without sufficient notice
